For health plans, health systems, pharma and EAPs
The Exposure Report: your documents, ranked by the measures they touch
A paid diagnostic of the documents your team needs to review first. We score your agreed templates, map each one to the Star, HEDIS, HCAHPS or FDA requirement it touches, rank them worst first, and show you reviewed fixes for the worst ones.
Send a blank template, not a copy with a patient's or member's details.

One report for each kind of organization
Health plans
Stars & HEDIS Exposure Report
Your member letters and notices, each mapped to the Star, HEDIS or CAHPS measure it touches, graded and ranked worst first.
How we help health plansHealth systems
HCAHPS & Readmission Exposure Report
Your discharge, education and consent materials, each mapped to the HCAHPS item, readmission measure or Leapfrog standard it touches.
How we help health systemsPharma
Risk-Text Exposure Report
Your risk statements, medication guides and patient-support pieces, graded for plain language and actionability, ranked worst first before MLR.
How we help pharma
Each report is short enough to forward to the executive who owns the measure. A document touches a measure; it does not set a score on its own.
How it is scoped

A sample diagnostic excerpt
Synthetic example. Not a member or patient document.
| Rank | Template | Measure it touches | Grade | Top finding |
|---|---|---|---|---|
| 1 | Prior-authorization denial letter | Member denial-notice rules; CAHPS getting needed care | 14 | The reason and the appeal date are in one long sentence. |
| 2 | Renewal notice (Spanish) | On-time Medicaid renewals | 12 | Never measured in Spanish; the deadline appears only once. |
| 3 | Discharge medication list | HCAHPS care coordination; 30-day readmissions | 11 | No purpose for each medicine; no number to call. |
This reviews the material. It doesn't establish comprehension, clinical accuracy or compliance.
Then prove it on your own members or patients
After the report, we help you run a split test on the measure you are paid on. Your data decides, not ours.
Pick one mailing or unit
One gap letter, one renewal notice, one unit's discharge sheets or one brand's patient guide.
Write the rule first
You pick the measure and the rule that decides, such as "expand if the revised group does better and the 95% interval excludes zero."
Split it
Half get today's version and half get the revised one, chosen at random. On a hospital unit, draw the weeks at random instead.
Your data decides
Your claims, call or quality data shows what changed. We don't need member or patient records.
| Organization | What you split | The measure you pick first |
|---|---|---|
| Health plan | One HEDIS gap letter, sent to half the members with the gap | Gap closure at 90 days, from your claims |
| Health system | Discharge sheets on one unit, revised in half the weeks, drawn at random | 7-day callbacks, and 30-day ED returns or readmissions |
| Pharma | Pre-MLR scoring on half of one brand's new pieces, drawn at random | MLR rounds per piece, and reviewer comments on readability |
Size matters. To see a rise from 30% to 33%, each group needs about 3,800 people. We work from templates only, and your member and patient data stays with you.
Scope your diagnostic
Tell us which documents matter most. We send a written scope with the templates, the fee and the delivery window, and nothing starts until you agree to it.

Questions
How long does the diagnostic take?
Scope, fee, reviewer availability and delivery window are agreed before work begins. A typical scope covers 15–20 templates.
Do you need member or patient data?
No. The diagnostic uses your template library: blank or sample documents. No member or patient data is required.
How is this different from a readability checker?
Readability formulas measure sentence and word length. We also score actionability (PEMAT) and IHA health-literacy criteria, measure Spanish natively, and keep a reviewer-approved record.
Will this help us pass an accreditation survey?
We don't award accreditation or promise a survey result. We produce document-level evidence (criteria, versions, reviewer approvals) that your accreditation team can include in its file.
What happens after the diagnostic?
You keep the ranked findings and reviewed examples. If you subscribe within 90 days, the fee is credited in full to your first year.
Can we test it on our own members before we expand?
Yes. After the report, we help you split one mailing or one unit. You pick the measure and the rule that decides before it starts, and your own data shows what changed. We work from templates only.
